********"as tight as you can by hand" assuming you are somewhat strong and implying that you use a ratchet and have a good position while you are doing it to maximize force exerted on turning the bolt.
P.S. put the header together out of the car, and then raise the whole thing in from the bottom. This makes tightening the midway flange bolts easier.
Also, remove the O2 sensor from the old exhaust manifold (same thing as header) when it is out of the car, that way you can use a big open-ended wrench.

I'm not too knowledgeable on the transmission's insides. I know what the parts are and stuff like that, just not really all that much. From what I know, i'd say that if it's bad, you will need to change it.
If you want to try to see if it will go away some other way, try draining and refilling the transmission fluid. Use Genuine Honda MTF. It's like $6-7 a bottle at the dealer and you need 2 quarts. It is very simple to change and well worth it. If it doesn't fix it, it'll at least make your transmission probably rev more freely.

Yea, it's on the side of the transmission. It doesn't have a hex head, it has a 3/8" socket wrench square hole on it.
many people use 10W-30 or 10W-40 motor oil, but the Honda MTF has an additive that is specifically for helping the syncros, so i recommend it.
