There's supposed to be a big nut and sleeve like thing on the back which holds it in. If that part hasn't just unscrewed and slid down the wires, a previous owner must have put it together without it.

You would take the whole panel out of the car, unplug the wires, and work on it from behind. Once the lighter is securely attached to the panel, plug the wires in and replace the panel into the car. Look for screws on the sides of the panel wich hold it in.
