Gas coming out of the fuel cap

Default Gas coming out of the fuel cap

Had my 90 Honda Civic hatchback in to be smogged and the gas was coming out of the fuel cap. Also there is a strong fuel smell in the car when driving. Any Ideas?

For the smell while driving, look closely for leaks on the fuel filter, line, etc. under the hood. The three-way valve (which I think is inaccessible without removing the gas tank) and charcoal canister control pressure in the tank.
