I don't know much about this kind of stuff, but my '98 DX does almost the same things. My 1-2 shift is fine most of the time, 2-3 slips unless I ease of the gas a bit right as its shifting and 3-4 is as smooth as it can possibly be. Also, mine feels like it disengages when I let off the gas. In fact, mine will actually SLOW DOWN when I'm coasting down a hill. I was told by two different transmission specialist that my transmission should either be rebuilt or replaced. Sorry I can't help you out though.

Drain and refill the transmission twice with Honda or Honda-recommended ATF. Drive the car a short distance between the first and second drain and refill.
