Any vacuum or intake air leaks?

Is the FITV clean and adjusted?

Is the throttle cable adjusted too tightly?

The test to check for air flow through the side holes is important. Once warmed up, the FITV one should be completely shut off, and you should be able to stall out the engine by blocking the IACV / air screw one. Excessive air getting into the intake manifold by any undesired path will increase the idle speed. Check also for missing vacuum hoses or a leak in the brake booster.
