Default 03 EX shifting probelms

Hey everyone. I have a 2003 Honda Civic EX has about 95k on it. Seems to not be shifting perfectly, at first I guess I thought it was normal, going from a Ford to a Honda, it was just different, but now I know something is wrong. When I go from a stopped position, sometimes, not all the time, going into 1st, 2nd, and 3rd gear, it sounds like it goes into neutral and THEN goes into gear, like its falling out of gear or something. It happens alot with the 3rd gear, but it doesn't happen all the time. Any suggestions on how to fix or find out the problem? I had a transmission flush done to it, and that did not work. Thanks guys.

are you pressing evenly on the pedal, or are you moving the gas up and down as you accellerate?
