Post 1998 manual honda civic problem

i've tried just about every forum trying to figure this out, hopefully someone can help me out here. okay. the car starts fine and starts out 1st - 3rd just fine. the second i hit the 4th gear and push on the gas it dogs like bad. shakes the entire car and the rpms jump up and down like its going to die and then revs up past about 6k even if i'm only doing about 45mph. someone mentioned earlier the IAC - but thats just for idling? the CEL came on but i dont want to drive it to get a diag done because i'm pretty sure i'll get stuck somewhere. any help would be appreciated thanks.

IAC does not cause problems driving, just idling. When the engine revs up but the car doesn't go proportionally faster, the clutch is slipping.
