Default 2006 Honda Civic Hybrid IMA battery acting weird

I just bought a used 2006 Honda Civic with about 120k miles on it a few weeks ago and now when I drive the IMA stuff isn't working properly. Whenever I'm driving the battery is always at 2-3 bars, I start driving and the assist/charging bars don't move at all for a while then randomly the charging bars fill up to about 3-5 and back to nothing. After a little while, the battery will suddenly say it is completely charged and the IMA features kick in and the assists bars start moving like the charging ones where it only does 3-5 bars at best before randomly going back to showing none. After all, that's happened in the span of a few minutes the batteries level quickly drains to 2-3 bars in the span of a few seconds and the cycle continues.

So basically
  1. IMA battery sits at 2-3 bars
  2. while driving the charging bar nor the assist bar move for a random amount of time
  3. eventually charging bar moves but it only goes up 3-5 bars for random amounts of time
  4. randomly IMA battery is full
  5. While the battery is full, the Assist bar randomly moves but only up to 3-5 bars for random amounts of time
  6. randomly IMA battery drains from full-back down to 2-3 bars in the span of a few seconds
  7. Go back to step 1.

Is this the sign of a faulty battery? a poor connection somewhere maybe? bad sensors? The system worked perfectly fine during the test drive and the first 2 weeks and supposedly the battery lasts a long time and was replaced not too long ago.
