If you can, find someone that will sandblast your wheels to get all the dirt, paint, etc. off of them. Once that's done, use a degreaser to remove any residue, oils, etc. that's still on the wheels. Then spray it with self-etching primer and let it cure.
If you can't get them sandblasted, get some fine-grit (maybe 500 grit?) sandpaper, and smooth out the imperfections and rough up the existing paint. This will work okay for primer, but not as good as the self-etching stuff.
Once the primer is on, hit it with a couple coats of white, then do a few coats of clear.
If you really want to get ****, go over the clear with 2000-grit sandpaper to knock down the orange peel, then find your favorite car polish and wax, and bring the shine back.
It's a lot of work, but it'll look great and last a long time.
