1990 civic si engine issue

That actually sounds like knocking / detonation inside the engine, seriously not good, don't drive with it like that. Check the timing marks again. You should remove spark plug #1 and probe in the cylinder to confirm the crank is at TDC. The crank pulley could have been put on without the key and then of course the marks on it will be wrong.
So tdc means that cranckshaft marks are aligned and the first cylinder is all away up? How should is check the piston, stick something inside of it?
Hopefully he didnt fu** my engine then. The guy is a mecanich that works at toyota dealership so is not a backyard one.

TDC means the #1 piston is at the top of its stroke, that's what the cam is timed off of.
Bring the cam to TDC, then pull the plug out of the #1 cylinder and put a straightened clothes hanger down until you touch the top of the piston. It shouldn't go down very far at all, maybe an inch or two. Not totally sure how far it's supposed to go down. I would just rotate the motor counter clockwise a bit more (from the crank bolt) and see if the hanger starts going down. If it continues to go up the timing is off. Could be the belt being off a tooth or him forgetting the key in the crank pulley.
If he did forget the key on the crank pulley that would cause the crank pulley to slip on the crank and mess up the timing.
