catalytic and exhaust manifold, one piece?
if you get the regular header, find a universal cat and have it welded in the exhaust somewhere. An ebay $30 special will NOT do the trick. An OEM cat would be good, but a Magnaflow one would do it too.
If you move the cat, you also have to move that secondary O2 sensor so it is still right after the cat.
Some smog laws are so strict that they will not pass a car if they see that there is any sort of modification to the exhaust system especially before the cat.

Get a megan or dc header and try to find an OEM ex cat. I have the same problem, but mine still passes smog for now. I'm trying to find an ex cat somewhere. Only thing you will have to do is extend the secondary 02 sensor wires and get the cat welded on.
my 01 lx d17 is all one piece. i put dc sports header on it and im going with a summit racing universal fit high flow cat. the only drawback is ill have to have an o2 sensor bung welded after the cat and extend the wiring, but im getting a custom bent 2.25" catback made up. will probably end up being cheaper than an ebay special, and higher quality.

I had the same problem on my 96 d16y7, first the cat was throwing a CEL, catalyst efficieny below threshold, so I took the heat shield off and saw that it was cracked. So I took it to honda, and suprisingly they told me that for some reason honda extended the emissions waranty on them to like 14 years or 150k miles. So they replaced the whole manifold for free. ANd to top it off, before they touched the manifold they had to perform some maitenence that included: oil/filter, distributor cap/rotor, and spark plugs and wires. All freeeeeeeee
