Unhappy a/c heater blower

I have a 2002 honda civic ex. the heater a/c blower motor has stopped working. I have 12 volts at the plug in to the fan. I can apply 12 volts with a jumper wire off the battery and the fan runs great. plug it back in to the fan and all I get a bump turn when you turn the key on. again I have 12 volts at the plug in harness. I ran the self diagnostic and it gave me a code of 12, which is a bad blower motor. help please, thanks.....Ronnie

It's almost certainly the power transistor. You have a code 12, motor is good, and power reaches the motor. The power transistor controls the ground side of the circuit.
