Failed Smog

Check the EGR system if that car has one. It could be a bad EGR valve, passages clogged with carbon, or the aftermarket header may not be allowing exhaust to enter the passage thru the head back to the EGR valve.

Otherwise probably the cat itself. Again aftermarket exhaust could be a problem because if there are no heat shields on it, the exhaust may become too cold on the way to the cat for it to work effectively.
Where is the EGR system? How exactly do you check it if it is in good working order?

In what state do you live in Bro...They must suck!!!Here in California when you do your smog test and ifit fails they tell you right away and even on the print out it says what caused it to fail.
I live in california. The reason why they didn't tell me what is causing the car to fail is because it was at a test only station, as the dmv requested.

What engine do you have? For 96 Civics, only the D16Y5 engine apparently has an EGR system:

don't know who is the jack a-ss who is doing your test. I always go to a test only station. He always tells me if something is wrong so I can repair the problem. That's the point to tell you whats wrong, so you can replace the bad parts. Go somewhere else where it's a test station only and ask them that you would like to get a pre-inspection is about 30 dollars and they should tell you whats causing it to fail

They told him that it failed because of high NOx at 15 mph, and all other tests passed. No test station is going to tell you for sure exactly which part of the car is defective, that's not part of the test.
