Default timing belt pulley key broke

1992 Civic LX, 1.5l sohc exiting expressway and car died. Turned out that the timing belt pulley woodruff key broke. This pulley is a one piece aluminum, the key is part of the pulley so the whole pulley has to be replaced. I replaced the pulley started up ran fine for about ten minutes and then died. Same thing happened. I am a little stumped, has anyone else ever experienced this before?

The bolt needs to be really tight; it holds everything together. Make sure you have the guide washers in place on both sides of the timing belt pulley.

So the person who did some work on this a month or so ago did not tighten the bolt enough and it took a month or so to break? Then the second one broke in 10 minutes?
